This may sound a bit harsh but I believe if you think that to keep him safe you need to make a 911 call, or have him do it himself, then do it.
I guess what I"m getting at - is you can not be his primary resource right now. It's good that he has reached out to you, and maybe there are some things you CAN do, like help get him to the care he needs, but you are NOT the right person to be holding all the emotional baggage that comes with that responsibility. You are NOT a therapist, you're not trained in this. That alone is enough. Put ontop your own troubles, you NEED to get what you need for YOU as well as for him.
So please, try to get him to somewhere he'll be safe, and if you can't talk to him about it PLEASE tell him that although you care about him, he needs to talk to someone professional about it so that he'll be ok, as you can not give him the care he needs.
And please, get yourself the care you need as well.
