It probably would be a good thing to assume that it is real and have a doctor examine him. If it is towards the front, it could be in the sinuses. I would at the least have a ENT examine him.
Does he have these symptoms only when manic? If so, it could be a hallucination. Let's not assume it's not real immediately but if he only feels it when manic, I wonder if it might possibly be a hallucination?
I hate it when anyone automatically thinks things are imaginary because someone has a mental illness. But we can't rule it out realistically either. Has he had a doctor's opinon recently since it spread?
