I have occasionally recommended to parents who are seeking a long-term care situation for their child that they could try putting together a program for home care as based on the Windhorse model but using local resources.

You could also try contacting them to see if they can make any recommendations in your area. You may have better options if you look at a private facility as compared to a public.
