No, the therapist I see didn't mention it in his paperwork, but I haven't seen any of my other healthcare providers give that either, such as my family doctor, dermatologist, Psych-NP, cardiologist, gynecologist, etc. They all have guidelines prohibiting sexual relations with clients, but I have never seen this mentioned with the paperwork I fill out when I first see them for treatment. Do you think it should be?
