It is a hard choice -- mental health or physical health. I think it is possible to have both. I, too, have gained weight since starting meds 5 years ago. I've gained 55 pounds and it isn't going anywhere. I've kind of resigned myself to being heavier, but at the same time, I can do a better job of eating more nutritiously, walking to keep my aerobic health going, etc. I'm realizing that as I get older, my ability to fend off weight gain is lessened; the meds were the straw that broke the camel's back so to speak. I've given myself permission to be heavier, but with that I know I have to work to maintain my physical health. I've taken to buying clothes that fit rather than trying to squeeze myself into clothes that simply don't anymore.

Your pdoc can help you with this by prescribing meds that are less likely to cause weight gain (although I think they all do to some extent and combinations of them even much more so). My pdoc only took me off of one me because of weight (zyprexa). I know in my case I'm going to have to work at keeping my physical health in line. Unfortunately, I'm not terribly motivated at this point, but my intentions are there.
