about 5-10 years ago my doctor changed from one agency to another. Being a survivor of child sex abuse I rarely like doctors let alone allow one to do my "yearly". Well This doctor I totally trusted and allowed him to do my exams so when the notice came in the mail he was leaving and I h ad to choose someone else in the agency I said NO WAY. I set an appointment with my doctor. and at that appointment I found out where he was going, what state coverage that agency accepted and he called the agency and asked if he can bring clients with him. Of course they said the more the marrier what business doesnt want more clients? So I went home called social services medical insurance department and told them my doctor was leaving one agency and going to another, since I was already established with him its just common sense to continue with him but I needed to switch my insurace coverage to the second of the two plans that the state offers their clients. Social services said no problem and I am still with that same primary physician. and if this happens again. I will again go with him even if I end up paying out of pocket.

Talk with your doctor and maybe you can do the same thing and instead of his/her leaving you can go along with the move.
