Was it an injection? then possibly it could be just the "shock" so to speak of having injected himself with something? Like fainting with needles. You don't necessarily have to be afraid of needles or anything. It's a vasovagal attack and your blood pressure drops so you black out and go horizontal so it's easy to pump blood to the heart/brain/vital organs. And afterwards you generally feel cold and clammy and malaised.
