this question is hard to answer because there are different cultures, different types of belief systems, different locations ie countries, states continents, different mental health systems. each one has their own guidelines and beliefs about the creation of DID.
Here (North eastern USA) where I am the mental health community believe it is caused by extreme abuse.

Again this question depends on location, personal beliefs, beliefs of the mental health community and what have you.
I no longer switch into alters but when I did it was because of a multitude of things.
Switching into alters is different for everyone. what causes my friend stacy to switch is not the same triggers for my switching before I was integrated.
other than the diagnostic criteria everyone with this is different and it affects each of us in different ways.
the best way to answer these questions is for you to go to a therapist or psychiatrist where you are, work with those people and discover through therapy why you have DID (cant remember if you are diagnosed or not, excuse the error if it isnt you that has DID), through therapy you can also discover what makes you switch into your alters.
