that's great to be writing down all the things so that you'll have a clear list to go from when you get an appt. as others have mentioned sleep and eating factor into this as well as medications.
have you considered looking up your posts here or any journal/blog you have from when you started meds til now to see how if there is any reflection of what you've been describing? we can see a noticable difference in our own posts from our recent medication change. just a thought for your consideration.
