Have you been checked for Thoracic Outlet Syndrome (TOS)? That could be a cause of the cold hands. And then if in general you have impaired circulation the feet are often cold because they're so far away from the heart.

I have TOS and my hands get cold and blue unless I am massaging someone (I am a massage therapist). Heh.
