Tell the person who is making the appointment that you have been feeling ill and need a check-up. If they ask your symptoms, tell them, "Chest pains, feelings of anxiety, sadness, and constant worry." Tell the nurse the same. Tell the doctor the same. They will take it from there, ask you lots of questions, take your BP and vitals and perhaps order some routine lab tests to screen for stuff. Having a panic attack feels very much like some symptoms of a heart attack. If it is all anxiety, they will recommend some treatment for you, to help you feel better. If there is a medical condition contributing, they will diagnose it and treat that too. It may be very difficult for you to admit this to the doctor's office, but, it will be familiar to them. Many of their patients will come in with similar symptoms and they are used to dealing with them. They will be very happy to help you, and it will be easy for you once you take that first step of calling. Let us know how it goes.
