I think it might include whether or not you are in a relationship (which might indicate an "issue"); remember, it asks a lot of questions where that choice is possible. I know that "eating disorder" includes the initial weight/height BMI rating, not just whether you have an agreed upon formal problem.

But, I believe not being in a relationship because you have difficulty forming or maintaining one would be for anxiety or other reasons; I think that might come out somewhere "else" rather than in relationships (since relationships would be about relationships and if you don't have one. . .)
