My psychiatrist regularly spends 40-45 minutes with me even though he schedules appointments for 30 minutes. One time, when I was in really horrible shape, he spent an hour and a half with me. When the secretary kept trying to tell him there were people backed up, he said, "They'll wait. We're not done here." He's a great guy.
