I think sometimes you don't intend to hurt the people you love, but your focus is just so narrow, so resrictedm that you can't see the effect your actions might have on others. Additionally, sometimes family or friends hurt because they notice that you are hurting, and you wish that you could just take away their pain, but you can't and you feel like all you do is hurt people. So you push them away, or try to hide how you are feeling and sometime that hurts them even more.
