I too use to be just like you are describing, I still am at times, but the first step I took in getting better was to define exactly what it was that I was so fearful of in the outside world - then I slowly put myself into that exact situation and tested my theory to see if how I was feeling was true or not.... and guess what I did NOT die like I felt I would.
I am still fighting the battle and I am now able to go out sometimes without the great anxiety and then at other times I fear and when that happens I go home and start all over.... life is getting good again for me.
