fresia, others may disagree with my opinion and i'm not sure what the program is about. when i had to fill out forms for my career employment i omitted my mental illness and meds, etc. even tho this info is considered private i felt i needed to protect myself due to ppl's lack of knowledge about my dx'es. i don't usually lie on purpose but that was my solution. another question i had re this-ever been treated in a psychiatric setting?, etc. i said, no. i added there is nothing that would prohibit me from completing the tasks of my job...in your case the program. you could also discuss with your doc re the med question. seems as tho he would/could omit the info too.
