Delegate some of the grandma care responsibilities to your teenagers or find some private care for her. Make sure you get enough sleep, take your meds, and start out part time and see how things go. I have a toddler and a husband and I actually find that getting out of the house for 20-25 hours a week helps my sanity. It makes me less frustrated with my son and it gives me structure to my week and a sense of purpose.
