Sounds like you have hit bottom so there is only one way now, up. Cheer up. I have had a similar experience in the past and one of the lessons I learned from that was that I was not alone. Its amazing how strangers are willing to help out in situations like this. I don't know if you are religious but if you are, you might consider talking to your priest or preacher.

When I was in such a position I sort of decided to see it as the beginning of a new life rather than the ending of my life. Your therapist going, moving to your sisters, all of this could be a chance for you to make a clean break and a new start. Who knows what moving in with your sister will bring. You might meet someone new, or an even better therapist or perhaps someone who knows someone might offer you a job. Its a chance to be a new you, to symbolically farewell bad habits, fears, relationships and start afresh.
