Not sure what issues you need to work on, firefly, but here on the east coast, some of the best residential/transitional hospital programs are located at McLean Hospital. I know they accept insurance in most of the programs but there is still a co pay and most people need to get preapproved. They are on the web so you can search them out. REality is though--it's the wealthy who are really able to have long term inpatient/residential treatment, not anyone else. Money speaks.
