Just a couple of ideas that have worked for me when I've gone through phases like this:
*Find an exercise buddy, schedule time to exercise either daily or every other day. If you're meeting someone, you're less likely to back out on it.
*Volunteer somewhere that has a set schedule for you (same time, same day, so there's no getting things mixed up)
*Set a schedule for yourself - put it in your phone with reminder alerts so that you will be reminded to eat, clean, leave the house, etc.
*Make plans with friends for lunch or dinner, so that you will have a reason to get cleaned up and go eat
