I think that my brother has that. He simply has a diagnosis of ADD, since Dr. Amen's types aren't that well-known/used/tested for. But the poor kid has a chronic sense of inadequacy, takes little pleasure in anything other than video games, feels that others don't like him, assumes that everything is going to go wrong, and occasionally suffers from depressed mood. He blames it all on his Concerta. :/ I suppose that according to the description you provided, he should be on Wellbutrin anyway.
I have bipolar II disorder (and very likely ADD as well, but I haven't been diagnosed). I can relate to most of that description myself, except that my depressions sometimes are more serious, hence my mood disorder dx. But I'm already on Wellbutrin, and it's helping neither my depressive nor my attention symptoms.
Overall, the concept of "limbic ADHD", along with "ring of fire ADHD" (another one of Dr. Amen's conceptions) seem to provide a link between ADHD and mood disorders. Its really very interesting!
