You are entitled to a copy of all your medical history. What you need to do is go to the places where you were treated and simply ask for a copy of your medical records.
Sometimes these records are kept away from the facility in another place and you may have to pay a fee to get copies made and these records be sent to you. However that is not always the case. And you are asked for proof of ID and to sign a form and then you are given your records.
