Thanks. I called them today and the medical records person talked to me. She seemed in kind of hurry, but basically told me I didn't need any appointment. I just walk in anytime and tell the front desk I need a Review&Request Records.

Then someone will come out and take care of me. And that it would cost me .75cents page, I think. Since I was being taken care of for a couple of years at this particular place it may be a steep bill. But it's important that I have it.
