Is there a low-cost or free clinic in your area? If so, might there be a doctor there who would fill out the forms for you? We have such clinics in my area, and maybe if there is a doctor there who will see you, your old doctor can fax over your records, and they could do the paperwork for the patient assistance program until you can get in to see the new doctors....

If you can somehow get on the patient assistance program for the meds, it can be a great help. I get one of my medications that way, and it works out fine. The most complicated part is doing the initial paperwork, but after that, it generally gets simpler. It's a big help - I hope it can work out for you.
