I didn't read the source you posted, and I know nothing about such things. However, I definitely think you need to start seeing doctors pronto and get their official evaluations as a start.....

Then, if they are willing to support you in your need for disability, you can get the appropriate forms to fill out.

That's the way I seem to recall I started the process anyway....
