Thanks for the encouragment.. So, today I ended up mentioning it to the PA..

I ended up bringing my son to the urgent care today b/c I couldn't get him into his regular Dr. The one PA that my T suggested I see was there. So, I was thinking about it the whole time and almost didn't do it. However, right before we were leaving I just brought it up. Told him the situation. He said, I could come and talk to him either tuesday or wednesday and go from there. So, I didn't get an rx today, but at least I know he knows and that he is willing to talk to me. So, that part is almost done.

Now, I have to call the MD the T is referring me, he will be the one who be treating me long term for my meds.
