WOW! I'm so sorry your P doc was so...blunt,
to put it mildly. If he's talking institutionalization, why aren't your parents involved. (Are you a minor?) Job wise there is a mandate called reasonable accomodations. It means if you tell your supervisor what your illness is (Don't have to go into detail) the company is required to make accomodations for you. If that means a leave of absence, so be it.
Not all hospitals are awful and you can choose where you go IF you do it voluntarily. Staff people who belittle you are IMHO the cruelest of people.
I would go to the next appointment, with a family member or close friend. Listen to what the P doc has to say and offer your own opinion. The advance directive is a great idea. Good luck. This will get better.
and it isn't worth suicide at all. You are more important than that.
