i have a document drawn up with my medical wishes (for the life of me it's name escapes me now)..so that in case of something i will not get such & such care if i am unable to speak for myself.

it covers my wishes in case of death, cpr, visitors, meds, etc. (living will? patients rights? gasping at straws here) you can appoint someone as next of kin or emergency contact that way & ask them to be notified or designate them to make all decisions for you.

i did it because i donated by body to a medical school after i died. this way there are no burials funerals etc for my friends to fret over or feel guilty about (since i have no family) & i know that with my myriad of odd health conditions i will be helping prepare others. but i am not t6o get an autopsy etc..i also do not want life prolonging procedures etc..

so maybe that is closer to what you need...
