Well, people who talk to themselves and who suffer from schizophrenia or multiple personality disorder are probably experiencing a symptom of their illness/condition, and the crude and ill-defined term "madness" has been applied in days of old. But if someone talks aloud to themselves in public (which I personally used to do), it is not necessarily an indicator of psychosis, it may be nothing more serious than engaging in a personal activity in an unsuitable social context.
Btw, I no longer talk to myself in public, but I have seen plenty of other people do it. Just the other day a guy rode past my balcony on his bicycle, and he was in full flow; having a very lively conversation with himself. I've seen people talking to themselves behind the wheel of their car, with no-one else in the car (ok, they may have been singing along to the radio, but I doubt it).
