Nicoleb2, did you read all the fine points on the form? I'm also involved in cub scouts and depending on where you are going and how long you are gone, some of the form can just be filled out by you and that is enough... for other occassions like going on adventure trips with the scouts, you need to fill out the whole form and have a physical filled out my doctor...

Just thought I'd bring that up...

I think "uncontrolled psychiatric disorders" would be the equivalent of "uncontrolled asthma"... controlled asthma is asthma that responds to the current medical treatment plan that I have in place... so I would think "controlled psychiatric disorder" would be one that responds to the current treatment plan... either one of those disorders could change quickly even if its currently controlled....
