Getting diagnosed depends, really. If you're more depressive than manic you could get a diagnosis of major depressive disorder. Many people do not get the bipolar diagnosis until they get pushed into mania due to taking anti-depressants. It all depends on the doctor.
In my case, I was never diagnosed with anything even though given meds randomly and showing symptoms throughout my life, including psychosis. I was put on Abilify by a psychiatrist long before I had my diagnosis. What got me diagnosed was I went in for my physical. My doctor asked me how I was doing recently and I litterally went from sitting on the exam table to on the floor weeping. Ding ding ding! That's a bit odd!
As for your second question, I do feel like that myself. It all depends. I think everyone wants to run away screaming sometimes depending on the circumstances and stress they are in. Also the dream-like state, I get that way off and on.
