Fears cannot go away if they are not confronted; you'll keep being chased by the same monsters in the same dreams until you turn around and say, "Stop chasing me!" The only way to do that is to acknowledge you are being chased by monsters.

One usually doesn't talk directly about the tests one takes for therapy, especially at first; they are just to give the therapist an idea of scope and strength of our symptoms. I would turn in the tests but, if your therapist wants to discuss them before you are ready, tell the therapist "no, not yet". You are in charge of how quickly your therapy confronts your fears (or "if" it does; you can not turn in the questionnaire and keep your fears if you want).
