Start simple with little steps. I've been using myfitnesspal and its really good for eating healthy and exercising regularly. I also use the nike app on my phone to measure the distance I walk and that helps motivate me. I find walking to be a good exercise and its easy to do regularly. Maybe aim to exercise 3 times a week at first rather than every day...

I think you need to go easy on yourself for not doing stuff when depressed. Its really hard to get motivated when your depressed and you need to reward yourself for everything that you do rather than beat yourself up over what you don't do. Some days getting out of bed is nearly impossible....
