Well, not to impose on Doc's definition (which by the way, howdy Doc. Been awhile ), but I'd define "extreme" as something that interferes with daily functions. Let's say for example that you neglected to do some duty (be it around the house, to her, professionally, w/e) because you were engaging in your fetish. THAT'S when it becomes a problem...or when/if you hit the point that arousal and the fetish are synonymous, meaning that you REQUIRE a specific sort of stimuli to become aroused.

To give an extreme example of the extreme, I saw a TV show of a young lady who was a furry who refused to leave the house without her full fursuit. Regardless of weather, function, occasion...regardless, had to be dressed up as...I believe it was a rabbit? Perhaps a fox...been awhile. Anyway, that's an extreme case of an extremity.
