I have had several leaves over the years at work due to sounds like the same conditon. My last leave, HR gave me 2 weeks to get the paperwork in.
HR will keep what happened with you, in her office, confidential.
I understand what your going through though, being worried about keeping your job.
I was diagnosed with Bipolar 7 years ago and still have my job, if that helps.
