It may be your medications as this is largely what they do, keep you in different parts of your own mind with chemicals. It's often in a conscious state as to help avoid the chaos of the subconscious.

Honestly it sounds like you do, you are just fluid between them so it appears as one. The subconscious is your dreams and emotions. The conscious mind focuses on your senses like touch, hearing, sight and sounds, analytical thought (math), etc. Focusing on one will draw you away from the other. The lower brain focuses on breathing, body temperature, EM field, etc. There are other layers of the mind as well, understanding these will aid in learning and controlling yourself.
