I'm not sure of the specific form you have, but I know what you're talking about. It sounds as if you'll be going through a full cdr review. I'm sure you're aware of this, but just in case, EVERYONE on disability will go through a review(s) at some point no matter what the disability. When you decide to stop seeing your doctor and taking meds you hurt yourself in several ways..
1. ss will request medical notes for the last 12 months of treatment. If there's no notes the only options that exist are to deny you or send you to one of their doctors. This is why you don't want to stop treatment, because often a decision can be made based on the notes of your doctor.
2. It automatically raises the question of just how disabled are you if you don't need any kind of treatment and you've admitted above to being non compliant.

s.s isn't going after a select few. it's law that everyone be periodically reviewed, because there's new treatments, new therapies, new advancements which may improve a persons condition. Also, the majority of people on disability keep their benefits. Regardless, the process is extremely stressful.
My advice would be this... be specific and detailed in filling out the forms and give examples (attach additional pages if you need to). If you have a hard time sleeping at night don't just say "I have a hard time sleeping" but explain how and be honest.
I also think you should make an appointment with your doctor so you'll have a more current assessment of your condition.
I don't think you'll automatically lose your benefits, but I do think you have potentially made things more difficult for yourself then what they needed to be. I'm only guessing, but I think it's likely that ss will want you to see one of their doctors for an exam. I do believe (not sure) you have the right to request your doctor to perform the exam, but you'd want to research that and then make sure your doctor would be willing to perform it.
