Have you talked with a Pdoc about your sleep issues? The inability to have restorative sleep makes it harder for you to process stress. Your fear of death indicates you're dealing with a lot of stress right now. The most important thing you need to remember is your body has to rest. You have to have adequate sleep. There isn't a reason for you to not have help. Trust me they have something out there that will help you rest without the phobia or nightmares.
