I would get a counselor to help with the thoughts all over the place and work on better sleep habits so sleep knows its place. Presumably you get tired and sleep sometime, just not at night when you'd like to? Night is quiet and our thoughts and worries can get the better of us if we have not trained them and/or if we have problems we are trying to avoid, our thoughts scatter, trying to help us stay away from the problems.
Grab a single thought and "work on it" and keep your mind focused on that thought you have chosen, or read an enjoyable book to give you something to focus on as you fall asleep. I read before bed, think about what I'm reading and enjoy those thoughts.
