Given the current political climate of fiscal restraint I don't think we need to worry about widespread institutionalization.
I'm also having a hard time envisioning forced treatment when there are so many who want/need treatment and can't afford it.
To me severe would mean when lives are endangered whether that danger is to the public at large or the mentally ill.
My thought on the general subject is that nothing much is going to come of this good or bad.
