This relationship isn't going anywhere and someone is going to get hurt WORSE than your concussion, which is bad enough! You should have left THEN. Anytime there is violence in a relationship, THAT should be the end of it.

You need to leave, and leave now. If this is the first time you've gotten violent, obviously this relationship is toxic for you. You need to get out before either he or you are seriously hurt. It's only going to get worse. Please see if you can stay with friends or relatives, and if you can't then find a shelter where you can stay until you get on your feet.
