Alternatively, you should describe the situation and send a letter to the authority that supervises and governs the activities of your T, asking if the T would be required to report. The concern is that the T might be able to track down the guy even if you do not provide his name. Say, if the T already knows whom you work for. If you go that route, you should request the name of the governing body from the T, without going into details.
