My heart goes out to you. I do question what the doctors mean by he needs to hit rock bottom. It sounds like he has. Have they done rule out test? It could truly be a physical conditon. He could have organic brain disorder. I know how trying it is for the "caregiver", but I wish someone was able to find a physical reason for the both of you.
