T notes are for the T's. My T didn't take notes but 20 years into therapy was calling my husband by the wrong name :-) I don't think things like that are necessarily important; what's going on in your T's life is important to them (more important actually) and I try to remember the adage, "Your therapy should be the most important thing in your life but it's not the most important thing in your therapist's life."
Sorry about the asthma problems at night. I have that too. When my asthma starts acting up (so I have to use my maintenance inhaler in addition to the Arbuterol) it shows up first at night. I hate suddenly waking up "uncomfortable" and having to sit up and get my act together for a bit. Good luck on your sleep test, hope it tells you something useful.
