Does anyone have any ideas on how i could approach my boss about this so she knows what is going on?

I had to leave the job before cause was in the hospital a lot and was really depressed. I don't want to leave because she gave it back to me and she even asked if I would be there all the days I work....I said yes.

Things are going good at work and I don't want to screw it up. I am already in a panic about tomorrow having to go back to work. I am scared out of my mind. I don't know how to get myself to work tomorrow.
