I had asked a similar question a while back and the prevailing wisdom seems to be that unless you think your diagnosis would cause an issue, then don't disclose. The question you encountered on the application is sometimes used as a sneaky way of getting around the American's with Disabilities Act that prohibits employers from asking such questions outright.
