I found that keeping an old fashioned spiral bound note pad and writing down the important things helped me to remember what I had to do. It works much better than typing things in on your phone. It may look a little flaky, but it's pretty effective. Sometimes a vitamin/mineral deficiency can exacerbate things. You should look into your diet and see what you're likely missing. Either work it into your diet or pick up some supplements.
Edit: Just noticed that you're from Ireland. Have you been checked to see if you're gluten-intolerant or celiac? The Irish have a higher incidence of gluten intolerance than most other groups. That can cause problems with attention and a lot of other health issues because it creates a situation where your body has a difficult time absorbing the nutrients it needs. A good portion of the people with celiac disease are unaware of the fact they have it.
