If you want to self harm then there are clearly strong feelings behind that. Wanting to talk about strong feelings isn't wrong - and probably a good reason why your therapist would be useful. Can you explain to them that you have an issue which you want to talk about but don't feel entirely comfortable doing so? Any therapist worth their salt would get the message and take the utmost care opening the lines of communication and broaching the subject thereafter. Good luck.
