Is the MD a Psychiatrist? None of the others would qualify under SSA rules. It has to be an MD or DO, and in the case of mental health diagnoses, I think they do ask for a specialist's opinion and not a general MD/DO. I'm surprised they didn't send you to one of their own medical evaluations if they weren't happy with the providers you went through. Definitely consult an attorney at this point.
