Your letter sounds good to me. You could add that you're entitled under HIPPA law, if you anticipate a problem. I wrote my T too asking for records but I was dismissed (I get that kind of reaction a lot). I had to hire a lawyer. Cost me way too much money $1,200. You'll probably get a better response.
